Use this form only to submit your thesis or dissertation for format check. The work will be returned to you with Acrobat notes on it, so you will need Adobe Acrobat Reader to view them. You must provide us with a current email address or we will be unable to contact you about your submission. When the format check is completed, you will be emailed with a URL from which you can download the corrected copy. Please complete all of the fields on this form. When you are finished with this form, click the "Next" button and you will be taken to the Confirmation Page and then to a page where you will select and upload the pdf file of your thesis or dissertation.
