Please complete all of the fields on this form. This information will be used for cataloging your thesis or dissertation at the UGA Library, so accuracy and completeness are very important. You must provide us with a current email address or we will be unable to contact you about your submission. When you are finished, click "Next" and you will receive a confirmation page. Click on the link on the Confirmation Page and you will be taken to a page where you may choose the pdf file of your thesis or dissertation and upload it for your Final Submission. (Required Field) The name of the file you are uploading for your Final Submission. The name must follow the ETD naming convention as outlined in the Help File. For example: smith_john_j_200108_phd.pdf (Your document will not be accepted if you fail to name it correctly.) Please be certain that the file name you enter in this field is EXACTLY the same as the file you are uploading.

Important! Before you click Submit make certain:

1. The file you are submitting is complete and formatted to your satisfaction. No modifications may be made to the final copy once it has been accepted as the official copy.

2. You Filled in all the fields in the form.

3. You and your major professor have completed, signed and submitted to the Graduation Office the Electronic Thesis and Dissertation Submission Form .

4. That you have named the file you are uploading correctly as outlined in the ETD naming convention.
